Patient Portal 2

Our online patient portal is an easy way for you to communicate with the practice from the convenience of your home. You can use the portal to request medication refills and non-emergency appointments, view your lab results, check appointment times, and send non-emergency messages to us. To access the portal, you can use either of the following options:

Method 1:

  1. Follow this link to the portal’s login page.
  2. Go to the “Create an Account” tab and click “No” when it asks if you have a Registration PIN.
  3. Follow the directions on the screen to complete the self registration process.
  4. Contact the practice to authenticate your account.

Method 2:

  1. Come in to the office to get your Registration PIN.
  2. Follow this link to the portal’s login page.
  3. Go to the “Create an Account” tab and click “Yes” when it asks if you have a Registration PIN.
  4. Enter your PIN and last name and click “Continue.”
  5. Follow the directions on the screen to complete the registration process.

For either method, you will get an e-mail asking you to confirm your registration. You will need to click through the confirmation link in this e-mail to finalize your registration and be able to log in.
